1.    That I am the permanent resident of the above said address.

2. That I am a student of Govt. Polytechnic College______________.

3.  That I have studying in 1^st year, Diploma trade Civil and my roll number is _____________.

4.    That my date of birth is 12.09.1995.

5.    That I require scholarship from College for the study.

6.    That previously I have never taken any scholarship from any college.

7.    That I will use the above scholarship amount for the study     purpose only and will not misuse the same.

8.    That I will obey all the terms and conditions of college at the time of getting scholarship.
